When should I get a home safety assessment?
A home safety assessment can be helpful before a problem occurs or after a change in health, mobility, or function.
You or a loved one may benefit if you:

What to Expect
One of our rehabilitation professionals visits you in your home and looks at you, your activities, and your environment together.
Rather than simply giving you a checklist, we observe how your home works for your individual needs and routines.
We may assess:
Getting Around Your Home
Walking pathways, flooring, rugs, furniture placement, assistive devices and obstacles.
Stairs & Entrances
Steps, railings, thresholds, lighting and getting safely in and out of the home.
Bathroom Safety
Getting on and off the toilet, stepping into the shower or bathtub, grab bars, seating and other safety needs.
Bedroom Safety
Getting in and out of bed, nighttime mobility, lighting and access to the bathroom.
Kitchen & Daily Activities
Reaching, carrying items, meal preparation and performing everyday tasks safely.
Balance & Mobility
How you walk, turn, transfer and move through your actual home environment.
Lighting & Visibility
Areas where poor lighting, contrast or visibility may increase risk.
Home Setup & Fall Risks
Clutter, cords, rugs, furniture placement and other environmental concerns.

Insurance and Pricing
We accept Medicare, Mass General Brigham Always Health, and most BCBS plans. For all other insurances, Rehab Health 360 operates as an out-of-network provider, meaning we collect payment upfront. We provide a superbill for potential reimbursement. Please check with your insurer regarding out-of-network benefits.
